I consent to Veterans Psychology using the information I provide to confirm my eligibility for Surf Life Saving NSW-funded support and to connect me with a suitable psychologist. My details may be shared with that psychologist or their practice administrator, who may contact me directly to arrange sessions. Veterans Psychology will verify my eligibility against a staff list provided by Surf Life Saving NSW, but my name and personal details will not be disclosed back to Surf Life Saving NSW. For invoicing and reporting, only de-identified codes will be used. I understand that my information will be handled securely and in accordance with Veterans Psychology’s Privacy Policy, and that I may withdraw my consent at any time (noting this may affect my access to Surf Life Saving NSW-funded sessions).
