I consent to Veterans Psychology using the information I provide to confirm my eligibility for organisationally funded support and to connect me with a suitable psychologist. My details may be shared with that psychologist or their practice administrator, who may contact me directly to arrange sessions. Veterans Psychology will verify my eligibility against a staff list provided by Veterans First Consulting, but my name and personal details will not be disclosed back to Veterans First Consulting. For invoicing and reporting, only de-identified codes will be used. I understand that my information will be handled securely and in accordance with Veterans Psychology’s Privacy Policy, and that I may withdraw my consent at any time (noting this may affect my access to organisationally-funded sessions). I also understand that my information will remain confidential unless disclosure is required by law or if there is a risk of harm to myself or others.
